About the Forms Element
Use the Forms Element to embed Custom Forms in proposals, Guest Portals, and Content Pages.

You can add Custom Forms to an Itinerary, Lookbook, Guest Portal, or Content Page by using the Forms Element in the Element Library.

Step-by-Step Instructions

  1. Hover between sections within any page. You will see a blue line with a blue plus button; click the blue plus to launch the Element Library.
  2. Select the Forms element.
  3. From the dropdown menu, search for or select the form you want to embed.

Where Do the Form Submissions Go

  • Responses appear under the Records section when you expand the File.

  • Responses will also automatically be attached to the guests contact profile:
    • If no contact profile exists, a new one will be created and the form submission will be automatically linked to it.
    • If a contact profile already exists, the form submission will be automatically linked to that profile. The system matches submissions based on the name and email address on a contact profile. When both fields match, the contact profile is updated with the new responses.
    • If the name or email associated with an existing contact is misspelled or entered differently on a form submission (e.g., 'Jen Agent' on the form but 'Jennifer Agent' on the contact profile), a duplicate profile will be created and must be merged manually.

Who Gets the Form Response Notifications

  • Form response emails are sent to the person who created the form.
    • Each submission automatically triggers an email notification to the form creator.
    • Responses are included directly in the body of the email.
    • The email subject line will read: You have a new form submission from [Contact Name].

Tips & Tricks 💥

  • To delete the Form Element, hover over it and click the trash can icon.

  • If a parent is filling out forms for their child or family, once the first form is submitted, they’ll see the option to easily add another by clicking Submit Another.
