When viewing a custom form submission, there are three different places where you can access the submitted information.

If a Form Was Assigned to a Specific File

1. Navigate to your Files Dashboard.
2. Expand the relevant File.
3. Click View Submissions next to the form.
4. Hover over the form submission.
5. 1. Click the eye icon to preview.
   2. Or click the download icon to save a PDF.
      

On a Contact Profile

• On the Forms tab of a Contact Profile you can either click the preview (eye) icon to see the full form or download a PDF of the form.
  

On the Submissions Tab of the Form

• You can either click the preview (eye) icon to see the individual form response or you can download a CSV file that includes all submissions associated with that form.

Best Practices to Have Clients Sign Off on Terms and Conditions

• (Option 1): Use the Terms and Policies element in the Form Builder. This will automatically pull in the Terms and Policies configured in your account settings, where they can be managed and updated.
• • It will show like this for the user:

• (Option 2): Add a text element in the Form Builder with the wording “Click here to view our terms” and link it to your terms. Then add either a checkbox or an eSignature field and set it as required if you would like clients to confirm acceptance.
• (Option 3): Add a text element in the Form Builder and paste your full terms and policies into the field. Then include a checkbox or eSignature element and set it as required if you want clients to acknowledge and agree.
• Or, use the Safari Portal built-in Guest Forms, which prompt guests to check a box agreeing to the terms and policies in your account settings.
