How do I create and manage Credit Card Authorization from forms?
Create, customize, and publish Credit Card Authorization forms in your Forms module.

You can collect credit card authorizations from your clients through a custom Form, allowing you to securely store their card details and use them to make payments on their behalf.

Add any other information you need alongside the card authorization — such as billing details, terms and policies, or an e-signature — and submitted information will automatically save to the appropriate Contact Profile.

To get started quickly, you can customize the ready-made Credit Card Auth sample available on the Forms page, or add a Card Auth field to any existing form.

And because the form link and authorization request do not expire, you can create a form once and use the same link whenever you need to securely collect card details.

⚠️ Important: Only Account Admins, or users with permission to create or edit Forms, can create and publish forms containing Credit Card Authorization fields.


Add Card Auth to a Form

Head to Forms from the main navigation. From here, you have two options:

Option 1: Start with the Credit Card Auth sample

  1. Click Create Form in the top-right corner.
  2. Select Credit Card Authorization to start with our ready-made sample form.
  3. Customize the form, fields, terms, and branding as needed.

Option 2: Add Card Auth to an existing form

  1. Open the form you want to edit.
  2. Find Card Auth under Inputs on the left-hand side.
  3. Add the Card Auth input to your form and configure its settings.

💡 We strongly recommend also including a required E-Signature with Email Verification, as well as your Terms & Policies. These provide additional evidence of the client's authorization and can help protect you in the event of a chargeback.



Set Up Your Form

Contact Information

The sample Credit Card Auth form includes First Name, Last Name, and Email, all of which are required. It also includes Billing Information, E-Signature, and Terms & Policies to give you a strong starting point.

E-Signature & Email Verification

When you add an E-Signature input, Email Verification is enabled by default.

You can turn Email Verification off from the element settings, but we strongly recommend leaving it enabled. Email Verification helps verify the identity of the person signing the authorization and provides additional protection against fraudulent authorizations and chargebacks.

Terms & Policies

Add a Terms & Policies input to clearly document what your client is authorizing.

You can select an existing template — including terms previously used with your Credit Card Authorization forms — or create new terms directly within the form.

Card Retention

Card details can be securely retained for up to 12 months.

To choose how long the card should remain available, click the Authorize Card button within the form builder to open its settings.

From here, you can also customize the Authorize Card button text.


Publish & Review Submissions

When your form is ready:

  1. Click Publish to make the form available to clients, or Save to keep it as a draft.
  2. Share the published form link with your client. The link and authorization request do not expire.
  3. View responses from the form's Submissions page.
  4. Credit Card Authorization submissions will also appear on the relevant Contact Profile under the Credit Cards tab.

Tips & Tricks 💥

  • Start with the sample. The ready-made Credit Card Auth form includes the recommended fields and is the fastest way to get set up.
  • Add Card Auth to any form. You aren't limited to a dedicated credit card authorization form — Card Auth can be added alongside other questions and inputs.
  • Keep E-Signature and Email Verification enabled. We strongly recommend requiring an e-signature with Email Verification to help verify who provided the authorization and reduce chargeback risk.
  • Include your Terms & Policies. Clearly documenting what the client is authorizing provides an additional layer of protection.
  • The Security Notice is automatic. Adding Card Auth automatically adds the Credit Card Information Security Notice to the bottom of the form.
  • The Security Notice can't be edited or removed while Card Auth is present. To remove it, first remove the Card Auth input and save the form.
  • You can still request authorization from a Contact Profile. Go to Contact Details → Credit Cards if you prefer to create an individual authorization request from there.
